Gloria Huh, Pharm.D., BCPS is the Supervisor of Transitions of Care and Ambulatory Care Services at Providence Health providing Transitions of Care services in both acute and ambulatory care settings. Dr. Huh is excited to explore innovative ways that pharmacists can be positioned in the health care system, particularly in decreasing readmission rates and improving quality of care, through addressing transitions of care throughout the continuum of care.
Dr. Huh has a variety of clinical interests, which include internal medicine, infectious diseases and oncology.
Dr. Huh obtained her Doctor of Pharmacy from Nova Southeastern University in Fort Lauderdale, Florida. She completed her post-graduate year 1 (PGY-1) pharmacy residency at Loma Linda University Medical Center in Loma Linda, California. She then completed a PGY-2 in Transitions of Care at Palomar Health in Escondido, California.
Dr. Huh’s research interests include hospital discharge medication management and skilled nursing facility patients.
